Insurance Message
We accept most insurance carriers. Please have your card with you at the time of your visit. If we file your claim, you are responsible for only the co-pay at the time of visit.

If we are not a provider for your insurance, if you are not insured or we do not file with your insurance company, you may be responsible for the entire charge at the time of service. Please check with the front desk to ensure that we are in-network with your insurance policy.

Please bring your child’s insurance card with you to each and every visit.

As part of our contract with insurance companies requiring co-pays, we are required to collect your co-pays at the time of your visit. If you have a high deductible plan we will collect $50 at each visit and bill you for the remaining balance after we receive an Explanation of Benefits from your insurance company.


Our office accepts cash, checks, Visa, MasterCard, Discover and American Express.
